Two years after the fatal Tamahere coolstore fire a "rogue" refrigeration engineer is suspected of installing a similar deadly hydrocarbon gas, in another plant, which later exploded in a technician's face.

Neither the injured technician - whose face and hands took the full brunt of the fireball - nor the company which contracted him were aware the refrigeration unit he was repairing was fuelled by the non-stenched gas.

A similar gas was implicated in the April 2008 explosion at Tamahere which killed Hamilton Senior Station Officer Derek Lovell.

The latest explosion, in May, has the New Zealand Professional Firefighters' Union and Institute of Refrigeration, Heating & Air Conditioning Engineers (IRHACE) fearing for their staff's safety and questioning the authenticity of some refrigeration engineers.

Carilion Clinic's efforts to systematically change the way it provides health care are coming into sharper focus.

The region's largest health care provider is pulling data and inking plans to help patients establish stronger relationships with their primary care doctors in an effort to keep patients out of the hospital and away from the emergency room. At the same time, Carilion's executives say they are closer to signing a deal with private insurance companies to establish a new payment system that rewards keeping patients healthy.

Since announcing four years ago it would convert from a traditional hospital system, Carilion has consistently talked about the need to reduce costs by improving the quality of medical care provided.

Carilion's leaders have thrown out numerous terms like "patient- centered care," "accountable care organization" and "clinic model." But besides hiring a lot of doctors and constructing new buildings to support the changes, Carilion hasn't shown how it intends to change its practices -- until recently, that is.

Now, Carilion is moving away from talking about its vision and philosophy and toward focusing more on specific steps, said Dr. Mark Werner, Carilion's chief medical officer.

"We've become more pragmatic," Werner said during an interview Thursday.

Some of that focus has been fueled by the national conversation surrounding health care overhaul and efforts to reign in the exponential growth in health care costs nationally over the years.

"It's helped us," Werner said of the new law overhauling health care. "It's allowed us to be more specific in our actions and given us more insights into the complexity of managing populations of patients."

Becoming accountable

Accountable care organizations are the hot topic in the world of health care policy, having been a focal point in the new health care law. The goal of this type of organization is to pay health care providers to work together to treat patients and prevent unnecessary illness and costs.

For Werner, an ACO isn't a noun but an adjective.

"We are accountable for the care of our patients," he said.

While there are some places that have established ACO models, Carilion is getting some attention for its work in the years prior to the new law to embrace the ACO principles.

Last summer as the debate over health care legislation reached a boiling point, Carilion was already in talks with insurance companies to arrange a new payment system.

Those talks were taking place because Carilion was named as a pilot site for studying ACOs. The study is being led by two nationally known health care policy groups, the Dartmouth Institute for Health Policy and Clinical Practice and the Brookings Institution's Engelberg Center for Health Care Reform. The intention is to find an alternative to the current fee-for-service payment model used in health care.

The talks with insurance companies, however, have taken longer than initially anticipated.

"With the possible exception of one [insurer], everyone is interested in making this work and working with us," said Don Lorton, Carilion's chief financial officer. "It is just much more complicated than the principles would suggest. The details are difficult."

Lorton said he believes a deal could be in place by the end of the year.

The concept revolves around saving money: The goal is to reduce costs by preventing things like unnecessary hospitalizations. If Carilion is successful, and the average cost to the insurance company drops, the question becomes how to share that savings with Carilion.

Focusing on specifics

Even though the payment piece is still being worked out, Werner said they are moving forward with the changes to patient care.

"We have the doctors, the data and the capability to get to where we've always said we wanted to get to," Werner said

A bogus caller has been jailed after returning money he stole from a pensioner - with Pounds 40 extra and a note containing his name and phone number.

Simon Crewe posed as a window cleaner to steal from the woman's home last month.

While the 85-year-old's back was turned, he made off with a purse containing Pounds 160.

But he felt so bad about it he sent her a letter of apology and Pounds 200 to make amends.

The 26-year-old, who has a string of previous convictions, immediately regretted the theft, Leicester Crown Court was told.

He was later contacted by the police and handed himself in.

Yesterday, he was given a twoyear-and-five-month jail sentence. He admitted the distraction burglary in Meadvale Road, Knighton, Leicester, on June 18.

At the time, Crewe, of Herrick Road, off Welford Road, Leicester, was on licence from a seven-year jail sentence, imposed in 2004 for four distraction house burglaries, with 47 similar offences taken into consideration.

The offences involved him calling at elderly people's homes, posing as an official from an electricity or gas company.

Alan Murphy, prosecuting, said Crewe was carrying a ladder when he called at the victim's home at 8.30am. She assumed he was her window cleaner and, as she left him outside to go upstairs on her stairlift, Crewe made off with her purse from a windowsill.

Mr Murphy said that eight days later he put a letter through the woman's door returning her money with an extra Pounds 40.

He said: "Crewe said he was sorry and left a contact number."

He later told the police he argued with his partner about money and went out looking for window cleaning work, but was tempted on seeing the purse. He said the victim was "a nice old lady".

Mr Murphy said: "He immediately felt bad but was too scared to return it for several days, having burnt the complainant's purse and bus pass. In fact, the letter returning the money upset her even further and left her confused." Sally Bamford, for Crewer, said it was an exceptional case and Crewe would have escaped being caught, but for his letter of apology identifying who he was.

She said he had changed his behaviour since his last sentence, and become more sympathetic to victims of crime.

Ms Bamford said: "It was an impulsive act and he did what he could to put the matter right.

"He handed himself in to the police and became emotional in interview, which had to be stopped because he was so overwrought.

"He takes full responsibility." Sentencing, Judge Sylvia De Bertodano said: "What makes this so bad is that someone elderly and vulnerable was targeted in their home."

She said custody had to follow "no matter how much remorse you feel".

Well, yes and it came in the form of an aphid called phylloxera, that fed on and destroyed vine roots. It came from America in the 1860?s, and by the early 20th century, had destroyed all Europe?s vineyards and most of the rest of the world?s as well. The solution was to graft the vulnerable European vine, vitis vinifera, onto the phylloxera-resistant American rootstock, vitis riparia, naturally a very expensive effort. The most immediate effect in Europe was that only the best sites were replanted and the total area under vines shrank drastically as a result. Elsewhere the havoc wrought was comparable and vineyard acreage is only now expanding to old original sites destroyed over a century ago.

The 20th century brought further change as science and technology revolutionised viticulture and wine making. But despite the chemical formulae and computerised wineries, the grape retains its magic and allure that attracts wine enthusiasts from all over the world.
